A normal tendon is a very organised structure. It is constantly changing in response to load. Small tears and disorganised fibres are normal and occur all the time, without you noticing as the tendon goes about repairing itself.

Overuse of a tendon can cause an accumulation of tearing, creating pain. Tendons can also be actively overloaded which causes tearing.
